At Generations Family Dental, we pride ourselves on exceeding your expectations for generations – but first, we need to let you know what to expect for your first appointment!
Prior to your first visit
Please click the link below. to download, print, and complete your Medical History forms. If you’re unable to do so, these forms will be available to you as you arrive at the office.
At your first visit
Please arrive at least 15 minutes prior to your appointment time. We ask that you bring your photo ID, insurance card (if applicable), and list of all current medications and medical allergies (if applicable).
Appointment Reservation Policy
If you are not able to make an appointment, we will be happy to get you back on our schedule! Please note, we require a minimum of two days notice for any appointment changes. If proper notice is not provided, a reservation deposit may be required to reschedule.
